Evidence Case 003 — Misattribution of Failure
Evidence Case 003 — Misattribution of Failure
Research Question
How might organizational failures be incorrectly attributed to individuals when system conditions are the primary cause?
Purpose
Demonstrate OSI’s ability to distinguish individual performance concerns from organizational condition failures.
Evidence Classification
| Field | Classification |
|---|---|
| Classification | Synthetic Demonstration |
| Purpose | Method and diagnostic-framework validation |
| Research themes | Capability; Trust; Flow; Accountability |
| Evidence level | Synthetic |
| Claims supported | The framework can represent system-condition explanations without automatically assigning individual responsibility |
| Claims not supported | Real-world prevalence; predictive accuracy; causal effectiveness; generalizability; evaluation of any real individual or organization |
| Next validation stage | Controlled synthetic variation, adversarial or confounded test cases, then empirical validation with governed real-world data |
Synthetic Scenario
A team experiences missed deadlines, inconsistent quality, and increasing operational friction. Management responds by focusing on individual accountability.
OSI Observations
Structural Conditions
- Responsibility boundaries are unclear.
- Authority is inconsistent with responsibility.
- Multiple conflicting reporting paths exist.
Predictability Conditions
- Expectations change.
- Communication is inconsistent.
- Operations are ambiguous.
Trust Conditions
- Confidence is declining.
- Defensive behavior is increasing.
- Initiative is reduced.
Flow Conditions
- Rework is increasing.
- Communication is being lost.
- Coordination failures are repeated.
Interpretation
There is no evidence in this synthetic case that individual performance alone explains the outcomes. The observed failures are more consistent with organizational condition failures than individual capability deficiencies.
Why This Matters
Misattribution may produce unnecessary disciplinary action, erosion of trust, loss of capable personnel, and persistence of underlying organizational problems. OSI recommends correcting system conditions before assigning individual responsibility.
Evidence Boundary
Synthetic demonstration supporting diagnostic research. It does not evaluate or rank an individual, establish causality, or authorize a personnel decision.
